As hospital lengths of stay decrease, there is a growing need for highly specialized post-acute care that is custom-tailored for patients who are recovering from serious conditions.

Take pulmonary-related conditions, for example.  Chronic Obstructive Pulmonary Disease (COPD), Pneumonia and Congestive Heart Failure (CHF) are respiratory-related conditions that have a high likelihood of re-hospitalization.  TCAH’s new Respiratory Care specialty program identifies potential candidates for respiratory condition management and offers proven clinical pathways and pulmonary oversight for early intervention, which improves outcomes and reduces the likelihood of hospital re-admission.

Thanks to a collaboration with Northwest Pulmonary and Sleep Medicine that involves a dedicated nurse practitioner who specializes in respiratory care and an early intervention strategy, guests breathe easier at TCAH, and respiratory-related hospital re-admissions are at an all-time low.
