Rest assured—the liaisons at Transitional Care of Arlington Heights (TCAH) can provide the necessary assistance every step of the way!
Registered licensed nurses or social workers by training, Megan Lauer, Libby Reinecke and Myra Valera start supporting TCAH guests and their families before they even step through the front door at the comprehensive post-acute care center.
“We help prospective patients, families, doctors and office staff make sure all the pieces are in place to provide a smooth transition from hospital to our center,” says Myra, Senior Nurse Liaison at TCAH. Myra and the TCAH liaison team help ensure continuity of care by assessing each individual situation, offering options and educating about disease management and post-acute care needs.
“Oftentimes people don’t realize how much help they are going to need after they leave the hospital, especially since hospital stays are getting shorter and shorter,” says Myra.
The liaisons often visit with patients and family at the bedside in the hospital to assess their recovery plan and provide support.
